How many points do I have to earn?
To qualify for the Accelerate Plan, the employee and covered spouse both must earn 10,000 points.
Who has to earn these points?
The employee and covered spouse have to both earn 10,000 points in order to qualify for the Accelerate Plan.
Covered dependents are not required to earn any points.

Employee Assistance Benefits
The Employee Assistance Program is provided at no charge to employees, spouses, or dependents covered by Group Basic Life Insurance.
Group Basic Life
Active full-time employees, their spouses, and dependents are eligible as defined in the North American Division Working Policy.
Each employee has:
$100,000 of coverage for employee
$50,000 for spouse
$10,000 for each dependent child
$750 for stillborn child

Long Term Disability
The Long Term Disability requires a minimum of 35 hours per week. This benefit is provided to eligible employees by your employer.

If eligible, you can purchase this coverage for you and your family members. With Accident insurance, you’ll receive payment(s) associated with a covered injury and related services. Accidental Death and Dismemberment
If eligible, you can purchase coverage in the following amounts as requested by you and approved by VOYA:
$10,000 to $500,000 for the employee
100% or less of the employee amount for spouse
$10,000 to $25,000 each for dependent children
Critical Illness Insurance
If eligible, you can purchase coverage amounts at $10,000 or $20,000 amounts. Critical illness insurance can provide a lump-sum benefit upon diagnosis that can be used however you choose – from expenses related to treatment, deductibles or day-to-day costs of living.

DisabilityFLEX
If eligible, you can purchase coverage in increments of $100, not to exceed 60% of your weekly earnings. If you are unable to work for a period of time due to a non-work-related condition, illness or injury, short term disability insurance offers financial protection by paying you a portion of your earnings.

Hospital Indemnity
If eligible, you can purchase this coverage for you and your family members. Hospital indemnity insurance pays a cash benefit if you or an insured dependent (spouse or child) are confined in a hospital for a covered illness or injury. The benefit is paid in lump sum amounts to you and can help offset expenses that primary health insurance doesn’t cover (deductibles, co-insurance amounts or co-pays), or benefits can be used for any non-medical expenses (housing costs, groceries, car, etc.).

Supplemental Life Insurance
If eligible, you may purchase coverage from $10,000 up to $750,000 in increments of $10,000 as approved by Voya.
Employee: Coverage not to exceed the lesser of 7X annual earnings or $750,000.
Spouse: Eligible for amounts from $10,000 to $250,000 (not to exceed 100 percent of employee election).
Dependents: Eligible for amounts from $1,000 to $25,000 (not to exceed 100 percent of employee election).
